Substance Use Disorder Professional/Trainee - Care Navigator - MATCH

Overview Substance Use Disorder Professional/Trainee - Care Navigator - MATCH Valley Cities Behavioral Health Care is seeking a SUD Professional/Trainee - Care Navigator to provide MAT (medicated assisted treatment) in addition to assessment, counseling, and therapy at various outpatient clinics in the greater Seattle and South King areas. The role works closely with medical providers and other clinical staff to screen, enroll, and provide Care Navigation / Case Management to individuals with Opiate Use Disorder. The candidate should be able to operate in a fast-paced environment, complete clinical documentation on time, and deliver quality, judgment-free client care. Key Responsibilities Assist with patient screening, program enrollment, scheduling of clinic visits, procedures and follow up. Identify client needs and develop a treatment plan. Provide SUD assessment and case management. Partner with community service providers to facilitate client transitions. Collaborate with pharmacies and Hub/Spoke team members to obtain prescriptions, medication orders and refills, and communicate as needed within scope of practice and applicable regulations. Ability to work independently and in a team environment. Ability to complete documentation in a timely manner. Complete data collection and enter data into electronic health records systems and tracking tools. What We're Looking For Education/Credentials: Associate's degree; SUDP or SUDPT credential with a minimum of 6 months of experience; ability to transport clients; current driver’s license and automotive insurance. Experience: Experience working with clients with substance use disorder; experience with case management or care coordination; experience working with clients with mental illness; CPR certification preferred (may be obtained upon hire).
