Key Responsibilities of the Registered Nurse - Home Health

• Observe, assess, plan, implement, and evaluate nursing support for well, chronic, or acutely ill individuals.
• Work closely with other health professionals/providers to address health needs.
• Coordinate communications and actions of various disciplines related to health and safety.
• Manage medications as prescribed by the physician and ensure related policies are in place and up to date.
• Report progress to the physician when the client's condition changes.
• Complete routine documentation and ensure the maintenance of health status records.
• Direct and supervise licensed practical/vocational nurses and provide staff training.
• Maintain communication with inpatient nursing and discharge coordinators.

Requirements of the Registered Nurse - Home Health

• RN License
• Oasis and HCHB experience required
• Experience in home care
