Job Title: Case Manager - Registered Nurse

Location: Fort Myers, FL

Setting: Hospital

Pay Range: 1718.91 - 1804.86

Schedule: 11:00 - 23:30

Contract Length: 13 weeks

Candidate Type: Traveler

Requirements: RN License; BLS; 2+ years of current Case Management experience; nursys must be on profile; DEGREE ACCREDITATION MUST BE ON PROFILE; Care Management certification (AMC or CCM) preferred

Responsibilities:

• Manage patient care plans in a hospital setting, ensuring compliance with clinical standards.
• Collaborate with healthcare teams to coordinate patient services and facilitate efficient care delivery.
• Float to sister campuses as needed, functioning as core staff.
• Maintain accurate documentation and ensure all required certifications and licenses are current.
• Follow facility policies regarding RTO and demographic profile requirements.
