What You Will Do

• Coordinate and oversee medical, dental, nursing, and clinic services for individuals receiving services.
• Monitor residents for signs and symptoms of acute illness, complications, and exacerbations of chronic medical conditions.
• Conduct weekly on-site nursing visits and complete health assessments.
• Review medical records and documentation, including MARs, seizure charts, meal plans, vital signs, and other health-related records.
• Provide nursing recommendations and coordinate or facilitate referrals for medical treatment when appropriate.
• Participate in clinical and staff meetings, providing nursing and medical expertise to support quality care.
• Supervise nursing-related tasks and patient care activities, including first aid, wound care, vital signs, and other delegated responsibilities.
• Monitor staff competency and provide guidance to ensure nursing-related tasks are performed safely and appropriately.
• Ensure medications are administered safely and in accordance with physician orders, applicable regulations, and organizational policies.
• Assist with medical training, AMAP classes, and annual in-service training for new and existing employees.
• Collaborate with program staff and other healthcare professionals to promote continuity and quality of care.
• Respond to health-related concerns and provide clinical guidance as needed.
• Participate in on-call telephone triage coverage and provide appropriate nursing direction and follow-up.

Minimum Qualifications

Required:

• Current New York State Registered Nurse (RN) license in good standing.
• Associate's degree in Nursing required.
• Valid New York State Driver's License.
• Ability to work independently and effectively across multiple program locations.
• Excellent clinical judgment and problem-solving skills.
• Strong verbal and written communication skills.
• Smartphone required.
• Must have completed the orientation period of 3–6 months in the current position, where applicable.
• Must be in good standing in the current position, where applicable.
• Must be current with all required mandatory trainings.

Preferred:

• Bachelor's degree in Nursing (BSN).
• Experience working with individuals with Intellectual and Developmental Disabilities (I/DD).
• Experience working within an OPWDD setting.
• Experience providing nursing oversight in residential or community-based healthcare settings.

Work Location

Port Washington, NY

Coverage: Two residential sites

This position requires travel between assigned program locations.

Work Hours / Schedule

Part-time weekdays (days varies)

This position includes on-call duties for telephone triage coverage.

Pay

$40,000–$45,000 per year
