A well-established skilled nursing facility (SNF) in Brooklyn is hiring an experienced MDS Coordinator (RN). This is a full-time MDS Coordinator role paying up to $175,000 a year, and we are looking for someone who has done this job before. Prior MDS Coordinator experience in a nursing home / skilled nursing setting is required.

If you are a Registered Nurse (RN) who knows the Resident Assessment Instrument (RAI) process, understands Patient Driven Payment Model (PDPM) reimbursement, and can run the Minimum Data Set (MDS) schedule without hand-holding, this MDS Coordinator position is a strong opportunity to be paid well for what you already know. The MDS Coordinator here works closely with the Director of Nursing (DON), therapy, and the interdisciplinary team, and is treated as a key part of the clinical leadership group, not an afterthought.

What You'll Do

• Complete and submit Minimum Data Set (MDS) assessments accurately and on schedule, following the Resident Assessment Instrument (RAI) manual

• Coordinate with nursing, therapy, social services, and dietary to gather documentation and ensure care plans reflect each resident's assessment

• Manage the assessment calendar, track Assessment Reference Dates (ARDs), and keep Patient Driven Payment Model (PDPM) reimbursement accurate

• Review clinical documentation for completeness and support the team on quality measures and survey readiness

Qualifications

• Active Registered Nurse (RN) license in New York State

• Prior experience as an MDS Coordinator in a skilled nursing facility (SNF) / long-term care (LTC) setting is required

• Working knowledge of the Minimum Data Set (MDS), the Resident Assessment Instrument (RAI), and the Patient Driven Payment Model (PDPM)
