RN Diabetes Educator

Role: RN Diabetes Educator Responsibilities: Provide expert, patient-centered education and care coordination for individuals with diabetes. Assess learning needs and develop personalized self-management plans. Deliver one‑on‑one and group education on glucose monitoring, insulin use, nutrition, and lifestyle modifications. Collaborate with an interdisciplinary team to support quality improvement and innovation. Contribute to improving health outcomes, especially in rural and regional communities. Qualifications & Skills: Nursing licensure, strong patient education abilities, knowledge of diabetes management, excellent communication skills, and teamwork orientation. #J-18808-Ljbffr Alpha Home Health and Hospice
