We're expanding our team and seeking experienced Licensed Vocational Nurses (LVNs) and Registered Nurses (RNs) who thrive in a collaborative, innovative environment. Key Responsibilities

• Provide compassionate, high-quality home health care under the supervision of a Registered Nurse, ensuring seamless continuity and safety for patients.
• Collaborate with interdisciplinary teams (physicians, therapists, and social workers) to develop and implement personalized care plans.
• Conduct assessments, administer treatments, educate patients/families, and monitor progress in dynamic home settings.

What We're Looking For

The ideal candidate brings:

• Active Texas licensure as an LVN or RN (per state requirements).
• Proven home health experience with a focus on patient-centered outcomes.
• A compassionate, positive attitude paired with professional drive and creativity.
• Strong technological skills for electronic documentation and telehealth tools.
• A fun, team-oriented spirit that fosters collaboration and joy in high-impact work.

• Experienced with Axxess (preferred)

Our field-based team of skilled clinicians provides life-changing service to patients in their own homes, located throughout the greater Chambers, Liberty, and Harris counties. COVID-19 considerations:
PPE equipment is provided. All staff making visits are required to wear N95 masks and gloves in all patient homes. If the patient showing respiratory symptoms gowns and eye protection will also be worn. All PPE is provided
