Role Overview An Advanced Practice Practitioner (APP) – Market Floater is a licensed and board‑certified nurse practitioner or physician’s assistant assigned to Longevity Health Plan members for the Institutional Special Needs Plan Model of Care. APPs float within a state market to fill vacancies, absences, or other staffing needs. Travel schedule and frequency are agreed upon with MDCS and the new hire for each market. APPs requiring regulatory oversight are hired based on available supervisory physicians in each state. Key Tasks And Responsibilities Provide collaborative primary care and services to a panel of Longevity Health Plan members, treating in the most appropriate setting based on member goals of care. Deliver the Longevity Health Plan Model of Care, including advanced care planning and treating in place. Facilitate clinic operations functions such as prior authorization, Part A and B services, medication reviews, and quality gap closure. Serve as a trusted advisor to members, families, and nursing home staff, educating teams on member needs and changes in condition. Communicate effectively with members’ PCPs, loved ones, and care team members regarding care needs, goals, and treatment plans. Balance member caseloads and market goals while supporting team needs. Promote Longevity Health Plan to members, families, nursing facilities, and other stakeholders. Demonstrate autonomy and accountability for practice. Establish meaningful relationships within the nursing home environment. Maintain a customer‑service orientation focused on high‑quality care. Perform related duties as assigned. Requirements At least 6 months experience as a Nurse Practitioner or Physician’s Assistant, with prior experience in geriatric, hospice, or palliative care, or a new graduate NP/PA supervised in the LHP New Graduate Fellowship program. Strong desire to work in the nursing home environment. Ability to travel statewide as directed. Proficiency with computer and technology. Excellent communication and interpersonal skills. Self‑directed yet collaborative team player. Advanced clinical skills in assessment, diagnosis, treatment, and prescribing for chronically ill patients. Independent practice capability, critical thinking, and organizational skills. Master of Science in Nursing from a nationally accredited program or equivalent Master’s Degree. Credentials & Coverage State‑unrestricted Advanced Practice Nurse or Physician’s Assistant with board certification (AANP, ANCC, AACN, or NCCPA) in the appropriate specialty. DEA License. Active national provider number (NPI) and active Medicare provider number. Valid state driver’s license with good driving record. Auto insurance coverage required by state of residence; proof must be provided upon hire and maintained thereafter.
