Certified Diabetes Educator
Geisinger Health System
Geisinger is seeking qualified diabetes educators to provide diabetes self-management training (DSMT) and CGM education in an outpatient clinic setting. You will assess patient knowledge, develop individualized care plans, and collaborate with a multidisciplinary team to improve outcomes. Responsibilities include teaching essential self-care behaviors, reviewing labs, documenting care, and mentoring new staff. A hybrid schedule offers some telemedicine opportunities after training. #J-18808-Ljbffr
