This is a full time position (30-35 hours/week) at our Concord clinic located on Poplar Tent Rd. Position Summary CVS Health is seeking a Nurse Practitioner (NP) to join our Primary Care team. In this capacity, you will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for the entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role will report to the practice manager. Responsibilities Primary duties and responsibilities: Patient-Centered Quality and Safety: Use knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional care to provide patient care. Perform history and physical assessments that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical symptoms. Provide patient counseling including pregnancy prevention, STI prevention/safer sex practices, contraceptive care counseling and medication management. Counsel and educate patients and families on health maintenance and respond to patient care inquiries. Evaluate and treat patients\' medical needs with safe and high-quality treatment. Document all patient care within an electronic health record according to current policies and procedures. Collaborate with internal and external colleagues to ensure high-quality care for all patients. Provide patient-centered care that promotes health, prevents illness, and treats disease. Refer to appropriate specialists and patient care components as appropriate; maintain patient confidentiality per PHI and HIPAA standards. Communication and Interdisciplinary Collaboration: Collaborate with the clinical care team to deliver high-quality, patient-centric, longitudinal care; participate in and, in some cases, lead clinical care team huddles; communicate effectively with team members and health-related agencies; participate in after-hours on-call rotation as needed; foster mutual respect and trust within the team. Healthcare Environment Management: Resolve conflicts using appropriate communication and management techniques; maintain positive relationships with team members; manage clinical and non-clinical duties efficiently; adapt quickly to new models of patient care; emphasize cost containment without compromising quality; maintain professionalism and self-awareness; assist with hiring, development, and evaluation of the primary care team; complete tasks for clinic operations including following up on lab results and receiving incoming clinical information. Education and Experience Education: Completion of master’s degree level Family Nurse Practitioner with current National Board Certification and State licensure to practice in the Advanced Practice role required. Required Experience: A minimum of 1 year experience as a Nurse Practitioner required; Family medicine or primary care experience preferred. Internal applicants with less than 1 year experience are eligible with successful completion of the CVS Minute Clinic Primary Care Training Program. Active Advanced Practice Registered Nurse, Nurse Practitioner license in good standing. Current National Board Certification in Family Practice. Skills and Abilities Working knowledge of quality measurement in chronic disease management. Self-motivated, prioritizes and solves problems, takes initiative, and advocates for patients and practice. Effective verbal, written, and electronic communication skills. Excellent organizational skills and ability to multi-task. Initiative, problem solving, adaptability, and flexibility. Ability to work remotely in a clinical care team culture and without direct supervision. Proficiency with information management and technology; willingness to obtain multi-state licensures; ability to collaborate with professionals to ensure quality patient care. Working Environment Regular interaction with patients remotely; ability to sit, operate computer and phone, communicate clearly, and interpret patient responses. Specific vision abilities include viewing computer screens and electronic devices. Pay and Benefits The typical pay range for this role is: $41.85 - $90.13 per hour or base annual salary, depending on experience, education, geography and other factors. This position is eligible for CVS Health bonus or incentive programs in addition to the base pay.
