Coordinate seamless care transitions for patients, managing specialized consultations, inpatient admissions, and collaborating with case management for successful discharge planning. Lead or actively participate in continuous quality improvement initiatives to enhance the efficiency and cost-effectiveness of care delivery within the clinic and the wider system. Maintain complete and timely patient documentation and electronic medical records to ensure compliance, accurate records, and support the financial integrity of the practice. Advanced Life Support preferred. American Heart Association or American Red Cross accepted. Education Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
