You'll be responsible for coordinating direct care scheduling, monitoring tracking systems, and providing clerical support to the assigned team. Essential Job Functions/Responsibilities: Coordinate the SOC visit by interacting with the patient or their representative. Proactively plan and assign patients to clinical staff. Perform visual checks of forms for discrepancies and process discharge documentation in a timely manner. Maintain accurate logs and distribute reports to appropriate staff. Update weekly schedules for interdisciplinary team staff and notify the Clinical Team Manager of discrepancies. Verify visits provided against visits ordered and scheduled. Receive and relay messages for assigned interdisciplinary team staff. Prepare schedules for weekend and on-call coverage and order medical supplies as needed. Participate in on-call rotation for weekend coverage and answer phones after hours. Support the Agency's customer service program by maintaining a responsive and knowledgeable approach to customer contacts. Perform other activities as assigned by the Clinical Team Manager. Position Qualifications: High school graduate; college degree preferred. Proficiency in Microsoft Office. Exceptional organizational and multitasking skills. Excellent telephone and customer service skills. Strong work ethic and time management skills. Demonstrates patience, flexibility, and a cooperative attitude.
