This is FT temp, M - F 10AM - 6PM or 9AM - 5PM, starts immediately and will last 3 - 4 months.
The Senior Housing Program is a permanent supportive housing program located in San Francisco that serves primarily low-income seniors who have experienced homelessness and are living with complex and long-term social, mental health, substance use, and medical challenges. The supportive services team consists of a full-time Program Coordinator, two full-time Case Managers, and a part-time Public Health Nurse. Working collaboratively, the team ensures that residents have access to the resources, services, and support necessary to achieve and maintain housing stability. A central focus of the Housing Program is to support and inspire residents' independence, wellbeing, and quality of life. Through advocacy, education, care coordination, and person-centered supportive services, the program empowers residents to remain successfully housed, strengthen their self-sufficiency, and foster meaningful connections within their community.
Qualifications:
· BA or BS degree and 2 years’ experience in social work, preferably in the non-profit or public sector; or equivalent combination of education and lived experience.
· 2 years’ case management experience working with diverse populations.
· Experience working with and sensitivity to issues affecting older adults with complex and long-term social and medical issues including people recovering from the trauma of domestic violence, substance use, marginal housing/employment, and mental health concerns.
