Bachelor's degree preferred Valid driver's license with reliable transportation. Excellent communication and interpersonal skills. Strong organizational and time management abilities. What a Client Care Manager will Do: Client Care & Case Management Conduct in-home client assessments and develop individualized care plans. Coordinate the start of care and ensure services are delivered according to client needs. Perform routine supervisory visits and wellness check-ins. Maintain ongoing communication with clients, families, caregivers, and healthcare providers. Address client concerns promptly and ensure high levels of satisfaction. Care Coordination Collaborate with the scheduling team to ensure appropriate caregiver-client matches. Monitor service quality and recommend adjustments as client needs change. Support caregiver communication and promote positive working relationships. Assist with onboarding new clients and coordinating transitions of care.
