Gerente de Atención al Cliente (Bilingüe)

ChildNet Youth and Family Services está buscando actualmente un Gerente de Cuidados Bilingüe para nuestro Programa de Gestión de Cuidados Mejorada (ECM) ubicado en Riverside, CA. Bajo la dirección del Supervisor de Gestión de Cuidados Mejorada, el Gerente de Cuidados (LCM) proporciona servicios directos, así como coordina la gestión de cuidados y funciona como parte de un Equipo de Cuidados para el Programa de Gestión de Cuidados Mejorada (ECM). El LCM supervisa casos específicos, coordina beneficios de atención médica, proporciona educación y facilita el acceso de los clientes a la atención de manera oportuna y rentable. Responsabilidades Evaluar las necesidades de los clientes. Supervisar los Planes de Cuidados Individuales. Conectar a los clientes con servicios sociales. Coordinar con profesionales de la salud. Utilizar prácticas basadas en evidencia. Llevar a cabo actividades de divulgación. Realizar otras actividades relacionadas según sea necesario. Trabajar ocasionalmente en turnos nocturnos o fines de semana. Requisitos de idioma y experiencia Capacidad para comunicarse en inglés de manera clara y efectiva, tanto verbalmente como por escrito. Competencia bilingüe en hablar, leer, escribir y comprender español. Experiencia previa como Trabajador de Salud Comunitaria, Especialista en Apoyo entre Pares, Asistente Médico, o rol similar es un plus. Requisitos adicionales Se requiere una autorización médica previa al empleo, que incluye examen físico ocupacional, prueba de tuberculosis y examen de drogas negativo. Verificación de antecedentes penales mediante toma de huellas dactilares. Una función esencial requiere conducir; mantener una licencia de conducir de California válida y seguro de responsabilidad civil de auto válido.
