Support individuals in participating in community activities and recreational outings. Implement individualized service plans and behavioral support plans. Maintain accurate documentation and progress notes. Foster a positive and inclusive environment that promotes independence and self-esteem. Ensure the safety and well-being of people supported at all times. Qualifications High School Diploma or GED COVID vaccinated Valid New York State Driver’s License, willing to drive as directed by the employer Certified in CPR/First Aid Certified in AMAP (meds) Certified in SCIP-R (crisis management) Preferred Experience 1+ year working with people with developmental disabilities Experience in residential or health-related settings Strong teamwork and documentation skills Physical & Work Environment This is a hands‑on role—you’ll be actively involved. You need to be comfortable lifting, providing physical care, and working active days. You should have a strong sense of responsibility and reliability: caring for the people you serve and staying until the next team member arrives.
