The nurse provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Essential Duties and Responsibilities: Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care. Utilizes the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care while delegating to others as appropriate. Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease. Communicates effectively and professionally with patients, family, and all members of the Interdisciplinary Patient Care Team. Manages assignments within the Care Delivery Model of Modified Primary Nursing. Demonstrates effective allocation of material resources. Patient Experience: Role models the tenets of Relationship Centered Care through various facilitation actions. Considers the patient's values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care. Establishes and maintains a therapeutic relationship with the patient and family. Uses evidence-based practices to increase understanding of patients' perceptions of care. Initiates service recovery for patients and family members with concerns and escalates when necessary. Demonstrates caring, respect, compassion, empathy, and active listening. Supports staff in using evidence-based practices to enhance patient care perceptions. Conducts Purposeful Hourly Rounding to address essential patient needs. Quality and Safety: Practices safe and efficient patient-centered care, identifying barriers and opportunities for improvement. Demonstrates clinical skills and knowledge essential for quality and safety in the patient population served. Implements process improvement strategies based on evidence and procedures. Engages in formal and informal peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects. Contributes to quality and safety practices and required compliance measures. Implements principles of high reliability to identify and avoid high-risk behaviors. Escalates potential safety hazards or gaps following appropriate protocols. Utilizes institutional information technology in nursing practice documentation. Operations: Performs charge nurse duties as assigned. Coordinates with nursing management to ensure appropriate supply utilization and inventory sufficiency. Escalates resource deficits as necessary. Demonstrates patient-focused and cost-effective approaches to patient care. Documents in a thorough and accurate manner in compliance with required standards. Facilitates patient throughput on a shift basis. Professional Development: Contributes to an environment of care to support clinical colleagues and patients.
