Crisis Care Coordinator (Kendall County MH Center) HILL COUNTRY MHDD CENTERS IS OFFERING A $2,000 SIGN ON BONUS TO EMPLOYEES Help us help others: The Crisis Care Coordinator will provide comprehensive care, from intervention to continuity of care, to individuals experiencing a mental health crisis, in the county clinic as well as surrounding counties within the HCMHDDC catchment. These services may be performed virtually or in-person, in the community, jail, mental health clinic, and/or other locations for adults and children as mental health calls or referrals are received. The delivery of services will be based on the needs of the community and individuals experiencing a psychiatric emergency. The Crisis Care Coordinator is responsible for assessing individuals in crisis situations, developing individualized crisis treatment plans, including inpatient hospitalization referrals as needed, and providing crisis follow-up contacts. The Crisis Care Coordinator will develop Person Centered Recovery plans, in collaboration with the individual/families served, that support individuals in learning skills to reach their specified service outcomes and to attain/maintain individual recovery. The Crisis Care Coordinator will provide services as authorized in the Adult Needs and Strengths Assessment (ANSA), ensuring timely and accurate documentation of those services. This position collaborates with intra- and inter-agency staff to improve services for the community. This position will also require flexibility in scheduling due to the nature of responding to crisis calls. You will get to: Essential Duties and Responsibilities Ensure that all individuals on the case load have a current Uniform Assessment (at least 95% of all CANS / ANSA's are completed on time) Must provide collaborative documentation at least 95% of the time or no later than 24 hours after delivery of service. Maintain a composite performance score of 93 or higher Will deliver services using the recommended Evidence-Based Practice recommended by HHSC. Travel as needed, with the ability to arrive within (1) hour for a person in crisis in the clinic county. Assist other counties in HCMHDDC's catchment areas with crisis services as needed. Participate in the on-call rotation to maintain 24-hour crisis operations, to include weekends as needed. Maintain a caseload of individuals working towards recovery, providing skills training, psychosocial rehabilitation, case management, and other services directed to the individual's specific goals. Person Centered Recovery Planning is developed in collaboration with the individual/family served and any other person or provider requested by the individual/family. Work with local county law enforcement and/or emergency medical services in follow-up and crisis prevention in the field, within dispatch, and/or within the jail to fill community gaps. Protect and ensure client rights always You'll definitely need: A bachelor's degree from an accredited college or university in social, behavioral, or human services, with the ability to be credentialed as a Qualified Mental Health Professional - Community Services (QMHP-CS) A valid Texas State Driver's License. Current automobile insurance and the ability to travel when necessary. We'll also want you to: Guards' client confidentiality by being knowledgeable of and adhering to HIPAA regulations. Adhere to all Hill Country MHDD policies and procedures. Must become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification without interruption thereafter. Maintains CANS/ANSA certification.
