Physician Assistant, In-Home Visits

Responsibilities Conduct pre‑scheduled in‑home visits for Medicare Advantage, Medicaid, and certain commercial plan members. Travel primarily locally within the designated metro area, e.g., Springfield, MA. Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month. Execute a strong visit completion rate, pacing 35+ completed visits per month. Provide availability Monday‑Sunday, with the majority of visits scheduled between 8 a.m. and 7 p.m. Be trained and credentialed for all health plan clients and lines of business in the primary coverage state(s). Be accountable to Signify Health operational, clinical, and member experience KPIs and metrics. Conduct video visits as needed, maintaining training, affiliation, and compliance with video visit requirements. Maintain training and complete all diagnostic and preventive service (DPS) products as directed. Maintain all required supplies and diagnostic equipment related to DPS offerings. Maintain active state licenses in coverage areas (FNP; AGNP certification preferred for NPs). Maintain board certification. Maintain multi‑state licenses (preferred). Have experience with Signify Health In‑Home Health Evaluations or a similar format (preferred). Be flexible to travel locally and within licensed states as needed (up to 25%). Possess the physical ability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds of supplies. Accommodate field observations and/or ride‑along experiences as requested. Conduct visits that include medication and medical history review, physical evaluation, and additional diagnostic tests when ordered; not required to prescribe medicine, order lab tests, or alter treatment regimens. Qualifications Part‑time employee with hourly base rate plus incentives based on visits completed. Malpractice insurance coverage during Signify Health‑related activities. Board certification required; FNP or AGNP certification preferred. Active state licenses in coverage areas; multi‑state licenses preferred. Experience with in‑home health evaluations or similar format. Ability to travel locally and within licensed states up to 25%. Physical ability to lift up to 20 pounds and perform required tasks.
