Job Description
Title: Case Manager - Hospice Care

Schedule: Tuesday - Saturday, 8 am - 4 pm, on-call rotation required

Assignment Details:
- Guaranteed Hours: 37
- Contract Weeks: 13

Requirements:
- Current nursing license
- Knowledge of OASIS documentation requirements
- Experience with high-tech nursing skills, including IV use and complex wound care

Roles & Responsibilities:
- Assess patient and caregiver needs to develop and implement an evidence-based plan of care.
- Serve as Case Manager for assigned patients, managing case loads and visit times according to productivity standards.
- Document nursing care provided and ensure integration of information for the healthcare team.
- Educate patients and caregivers on medication, treatment options, and discharge instructions.
- Collaborate with interdisciplinary team members to coordinate comprehensive patient care.
- Participate in peer review audits and advocate for patient care quality.
