Licensed Practical Nurse (LPN) – Skilled Nursing / Long-Term Care

Position Summary:

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing and long-term care team. The LPN will provide high-quality, resident-centered care while collaborating with an interdisciplinary team to support the health, comfort, and well-being of our residents.

Responsibilities:

• Provide direct nursing care to residents, including medication administration, wound care, treatments, and vital sign monitoring

• Develop, update, and implement individualized resident care plans in collaboration with the interdisciplinary care team

• Perform and oversee routine resident care needs, including assessments, bathing, toileting, and activities of daily living support

• Monitor resident conditions and promptly report changes in health status

• Maintain accurate, timely, and thorough documentation of resident care, treatments, and progress

• Communicate effectively with residents, families, providers, and team members to promote coordinated care

• Support infection prevention practices and facility standards of care

• Participate in quality improvement initiatives focused on improving resident outcomes, safety, and satisfaction

• Promote a respectful, compassionate, and supportive environment for residents

Requirements:

• Current Licensed Practical Nurse (LPN) license in the State of Oklahoma

• Previous experience in skilled nursing, long-term care, or geriatric nursing preferred

• Strong clinical assessment and communication skills

• Ability to work collaboratively as part of a multidisciplinary team
