Description

• Conduct thorough evaluations of patients in inpatient or outpatient environments and develop personalized treatment plans
• Implement physical therapy techniques emphasizing mobility, strength, and balance for optimal functional outcomes
• Collaborate proactively with nurses, physicians, case managers, and rehabilitation specialists to achieve best patient outcomes
• Provide education for patients and their families regarding therapeutic objectives, discharge planning, and home exercise programs
• Maintain accurate documentation of evaluations, treatments, and progress in line with facility standards and regulatory requirements
• Engage actively in interdisciplinary meetings and care team rounds to ensure seamless care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (e.g., neurorehabilitation, geriatrics) are advantageous but not mandatory

• Prior experience is strongly preferred
