Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 26 weeks Shift: Day Hours per Shift: 10 Experience: Minimum 1 year of acute care experience License: RN license required Certifications: BLS preferred, ACM Certification preferred Must-Have: - Epic experience - Acute care experience - Ability to work weekends Description: The Case Manager RN will oversee discharge planning for patients. Candidates must possess an RN license. BLS certification is preferred but not required. ACM certification is preferred. Experience with Epic charting is required. The role involves working a 4x10 or 5x8 schedule with varying shift times from 07:00 to 18:00. Weekends are required; every other weekend or four weekend days in a four-week schedule. Candidates will float within the case management services to cover gaps as needed. Travelers may be called off for one 8-hour shift per two weeks. RTO requests are limited to one time off request per 13-week assignment for five days or less. No holiday time off requests will be accepted for auto offers. The facility is a Magnet facility with a total of 407 staffed beds. The patient population includes neonates, adolescents, infants, adults, pediatrics, and geriatrics. Daily census is approximately 324 patients. Collaboration with interdisciplinary teams, including physical therapy, social services, and others, is required. Knowledge of medical necessity and evidence-based guidelines is necessary. Candidates will be responsible for care coordination and conducting continued stay reviews. The facility requires that candidates have experience determining medical necessity using established criteria. This position is essential for maintaining compliance with health regulations and standards. Travelers must be willing to work major holidays as needed. Housing options should be explored in the area, as availability may be limited.
