Registered Nurse - Hospice

$1996.48/week -

Job Summary:

The Hospice RN provides comprehensive home nursing care to patients, coordinating and delivering services according to physician orders and agency policies. The role involves supervising home health aides and licensed practical nurses while ensuring quality patient outcomes in a home setting.

Responsibilities:

• Coordinate and direct home hospice care in accordance with physician directives and agency protocols.
  
  
• Supervise and evaluate the performance of home health aides and LPNs/LVNs.
  
  
• Conduct patient visits for start of care, wound care, follow-ups, and other clinical services.
  
  
• Document all patient interactions and care activities accurately within established timeframes.
  
  
• Assess patient needs, develop care plans, and adjust treatments as necessary.
  
  
• Travel up to 75 miles within designated territory to provide timely patient care.
  Qualifications:
  
  
• Valid RN license with current state licensure in hand.
  
  
• Minimum of 1-year recent experience in adult home health or hospice care.
  
  
• Proficiency with HomeCare HomeBase (HCHB) EMR system; Oasis experience required for RNs.
  
  
• Reliable transportation, valid driver’s license, and agency-required liability insurance.
  
  
• Ability to travel up to 75 miles and manage a patient caseload efficiently.
