Seven Healthcare are seeking a skilled RN - Case Manager for a travel assignment in Englewood, Colorado. This role specializes in Case Management within an acute care environment. The ideal candidate will work closely with a supportive team to manage patient care efficiently.

Key Responsibilities of the RN - Case Manager

Efficiently manage patient caseloads within an acute care setting
Conduct discharge planning assessments
Collaborate with a team of case managers and social workers
Engage in GOC conversations with palliative care knowledge

Requirements of the RN - Case Manager

Minimum 1-year current acute care Case Management RN experience
Experience in discharge planning and ED
