Job Description Licensed Practical Nurse Needed for full-time night shifts (Saturday to Wednesday) in Castle Rock, CO. Case available with patient requiring assistance in lifting and transferring. LPN rate $24-28/HR with sign-on bonus: FT Nights $3,500. Qualifications Valid LPN license in the state where clinician will practice. Current CPR certification. Willingness to provide in-home nursing care to infants, children, and/or adolescents. Graduate of an accredited school of nursing. Current unrestricted state license as a Licensed Nurse in the state of practice. Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures. State-specific requirements: South Carolina – 1 year of pediatrics experience; California – 1 year under current nursing license; Louisiana – 1 year as licensed nurse; Continuing Education as required by state. Six months of recent experience as a Licensed Nurse in a clinical care setting (preferred). Home health experience (preferred). Responsibilities One-to-one patient care in a home environment. Execute physician’s prescribed plan of care and provide compliant documentation in the system of record. Administer prescribed medication, treatments, and therapies. Conduct patient assessments and coordinate care. Health promotion, teaching, and training of family members. Provide bedside nursing care under RN supervision, following orders and maintaining confidentiality. Observe and assess patient condition, report changes to supervisor/physician. Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner. Participate in, implement, and update the nursing care plan. Take appropriate nursing action based on assessment to achieve expected outcomes. Prevent or reduce risk by intervening appropriately. Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act. Implement infection control measures per OSHA standards. Educate patient and family regarding disease process, self-care techniques, and prevention strategies. Maintain competence by participating in continuing education and internal learning management programs. Other duties – duties may change as notified. Benefits Health & Life Insurance.
