Weekend Clinical Intake Coordinator (LVN / RN)

Saturday & Sunday This position is exclusively for Saturday and Sunday shifts. About the Role This is a unique opportunity for a clinically experienced LVN or RN to step into a high‑impact operational role at the agency level. As our Weekend Clinical Intake Coordinator, you will serve as the central point of contact for our entire organization on Saturdays and Sundays, managing Kaiser referral intake, supporting field clinicians with clinical escalations, and ensuring our patients receive timely, seamless care coordination when our weekday leadership team is offline. This role is ideal for a clinician with home health experience who thrives in a fast‑paced, autonomous environment, communicates confidently with both clinical and operational partners, and can triage competing priorities with clarity and calm. You will be assigned a primary location; however, you may occasionally be asked to provide coverage outside of that area. This may include supporting vacancies, leaves of absence, higher patient census, or other client care needs.
