Physical Therapist

Swing bed and outpatient opportunity. Work 5 days a week, totaling 40 hours per week. The Physical Therapist provides specialized physical therapy services within the home health and hospice environment. Deliver comprehensive, patient-centered care while ensuring accuracy, safety, and compliance with all regulatory standards. Provide rehabilitative services to home health and hospice patients. Carry out skills that support treatment, rehabilitation, safety, maintenance, and education for patients and their families. Particular attention is given to the geriatric population. Work as part of a collaborative interdisciplinary care team. Plan and coordinate therapy services for individuals and families. Assess health needs and potential or existing hazards. Consult with or refer patients and families to appropriate agency or community resources as needed. Deliver treatment and preventive procedures under medical direction. Evaluate, plan, implement, supervise, and document care under the guidance of the supervisor or manager. Ensure compliance with established standards of performance and organizational policies. Perform comprehensive physical therapy assessments and develop individualized, evidence-based treatment plans. Supervise and coordinate care with Physical Therapy Assistants. Provide therapeutic interventions focused on mobility, strength, pain reduction, and functional independence. Collaborate with interdisciplinary team members to coordinate patient-centered care and discharge planning. Educate patients and caregivers on safe mobility techniques, equipment use, and home-exercise programs.

Experience: Strongly prefer 2 years previous experience as a Physical Therapist License: Licensed Physical Therapist in North Dakota Certifications: Current driver's license required and vehicle available for travel to patient's home
