We're a tech-first team so comfort working with new systems and tools will be an advantage. Also a huge plus if home modification work is part of your experience.We're recruiting now as we know it takes time to get credentialed with the plans we partner with so hope you're equally excited to get ahead of things and get introduced to Jukebox!Role HighlightsAnticipated Start Date: ImmediateCompensation: $120 per EvaluationEmployment Type: Contracted, minimum of 5 hours of weekly availabilityLocation: In-Home VisitsKey ResponsibilitiesConduct in-home assessments and functional evaluationsDeliver personalized, evidence-based recommendations for home modificationsEducate clients and caregivers on fall prevention and safe mobility techniquesEngage clients and families in collaborative care planningWork closely with our Director of Clinical Operations to share findings and support process improvement initiativesMaintain accurate and timely documentation through our electronic systemQualificationsDegree in Occupational Therapy from an accredited programActive, unencumbered OT license2+ years of clinical experienceExperience in home health, aging in place, or disability services preferredStrong interpersonal and clinical reasoning skillsReliable transportation and valid driver's licenseProficiency with technology and digital documentationAbility to work independently while collaborating with a remote teamMore About CompensationContractors are paid on a monthly basis, within 2 weeks of the previous month's end.We pay per completed evaluation - this includes:Travel to/from client's homeThe evaluation itselfDocumentationIf needed, we will pay additionally for extended travel - we will always discuss and approve ahead of time
