General Summary:

LPN provides nursing care to HIA Primary Care patients in coordination with the interdisciplinary team (IDG) that is consistent with the goals, definitions, philosophy, and purpose of HIA Primary Care. LPN will serve as a care coordinator to their assigned providers. Duties to include intake, assessing patient needs, care coordination, scheduling and other medical practice functions.

Essential Functions:

• Works in collaboration and continuous partnership with chronically ill or "high risk" patients and their family/caregivers, providers, facilities, clinics/hospitals, staff and community resources in a team approach.
• Promote timely access to appropriate care
• Increase utilization of preventive care
• Reduce emergency room utilization and hospital readmissions
• Increase comprehension through culturally and linguistically appropriate education
• Increase patient's ability for self-management and shared decision making
• Connect patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs.
• Assess phone calls with the ability to determine the urgency of patient, family, or facility request; if uncertain or require a level of expertise beyond LPN scope, check with RN or provider.
• Handle patient, family and facility advice calls, discuss test results, answer questions; if uncertain or require a level of expertise beyond LPN scope, check with RN or provider.
• Provides optimal care to patients with regulatory guidelines and LPN scope of practice.
• Assist with the identification of "high risk" patients and flag in the EMR.
• Medication management, including identifying potential medication issues, answering questions on side effects and medication conflict.
• Complete any prior authorizations for medications, DME, procedures, etc.
• Initiate/assist with DME, Home Health, home infusion/ PT/OT orders as directed.
• Completes and locks documentation on the same day as the contact/communication occurs as per the Medical Records Documentation policy.
• Record accurate and concise documentation pertaining to the patient history and phone conversation.
• Enter and follow up with orders from providers
• Implements and utilizes physician orders accurately and effectively;
• Complete, update and refill prescription requests accurately within a timely manner;
• Complete follow up phone calls for order completion; and
• Enter controlled substance scripts for providers.
• Prep patient chart(s) for providers
• Reconcile data received and/or collected from document received from facilities, clinics/hospitals or EPIC, especially for new patients.
• Diagnoses, allergies, medications, immunizations, surgical history, etc.
• Medication reconciliation
• Call facilities or home patients to get complete list; and
• Check on discontinue medications that should be removed.
• Verify vaccination records for patients on state registries, facility documents, or from EPIC
• Follow up on completed and update state immunization registries.
• Annual Wellness Visits
• Ensure we have all CMS required information for visit
• May need to complete HRA prior to AWV visit
• Collect, enter and update information in patient chart;
• Ensure all required consents are completed and in patient's chart; and
• Help answer clinical questions on set-up of patient chart.
• Monitor Athena clinical inboxes for providers
• Label forms, reprocess documents, check for correct facilities/pharmacies; and
• Recognized when urgent responses are required.
• Track monitor and schedule patient labs, tests or procedures ordered by provider.
• Various clinical and administrative tasks.
• Participation in IDG, team and organizational meetings as required.
• Demonstrates preparedness for IDG meetings and provides IDG updates to a teammate prior to any planned vacations.

Pay Range: Starting at $24.35/hour (based on experience)

Qualifications

Education, Certificates and Experience:

• Minimum of LPN degree.
• Minimum of 3 years nursing experience in clinical setting
• Strong clinical skills
• Strong interpersonal communication, counseling, and teaching skills
• Flexibility in scheduling; team player
• Familiarity with principles related to dying and death strongly preferred

• Current LPN licensure in North Dakota and Minnesota (or willing to obtain dual licensure up on hire)
• Current driver's license and auto insurance
