Position Overview Full‑Time Hours: 40 Hours/Week (Flexible Scheduling) Minimum 25 Visits/Week (7 Points/Day) Pay Structure: $95,000 - $115,000/year (Based on Pay per Visit Rates and Experience) Sign on Bonus: $5,000 Additional Earnings: $2,000 Quarterly Bonus for Maintaining 90% Average Productivity Coverage Location: Jasper, Indiana & Surrounding Areas (Including Washington, IN) Travel: Required + Mileage Reimbursement ($0.48/Mile) Work Setting: In-Home Patient Visits, Training On-Site at Our Local Office and Remotely Responsibilities Play a vital role in helping patients regain mobility and independence Deliver physical therapy services per physician’s care plan and state regulations Assess patient mobility, strength, and rehabilitation potential Design and implement personalized therapy programs Educate patients and caregivers on therapeutic techniques and equipment Collaborate with interdisciplinary teams to ensure holistic care Supervise PTAs and home health aides as needed Document progress and participate in discharge planning Qualifications Required: Active Indiana PT license Degree in Physical Therapy (Bachelor’s, Master’s, or Doctorate) CPR certification Valid driver’s license, reliable transportation, and auto insurance Recent TB test Preferred: 2+ years of PT experience Home health or community-based care experience Strong communication and organizational skills Ability to build rapport and lead with empathy Benefits Dental Insurance Vision Insurance 401(k) Mileage Reimbursement Referral Program Flexible Schedule #J-18808-Ljbffr
