Pharmacist - PRMC - DH

Overview The clinical generalist will provide pharmaceutical care services to patients at PRMC & Parkview Hospital Randallia in Fort Wayne, Indiana. Clinical generalists rotate through clinical and distribution services, with >60% of the month assigned to a clinical pharmacist role. Responsibilities Participate in multidisciplinary rounding, seven days per week. Oversee, monitor, prepare, dispense, and properly document patient medication therapy. Advise physicians on drug usage and control within assigned hospital locations. Provide drug information and patient counseling, and oversee dispensing processes and systems. Perform clinical practices such as dosing and monitoring of anticoagulants, antimicrobials (including vancomycin), total parenteral nutrition, patient‑controlled analgesia, and other medications. Mentor pharmacy IPPE/APPE students, five PGY1 pharmacy residents, and one PGY2 ambulatory‑care pharmacy resident. Qualifications Pharmacy degree from an ACPE‑accredited institution. Completed PGY1 pharmacy residency or equivalent hospital practice experience. Indiana pharmacy licensure or eligibility to obtain licensure. High level of clinical practice skills, enthusiasm for teaching, and strong interpersonal skills for interacting with students, health professionals, and patients.
