Occupational Therapist - Home Health

Multiple Locations in California (Area #1: Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City; Area #2: Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont; Area #3: San Ramon, Dublin, Pleasanton, Sunol, Livermore)

This is a 13-week contract position.

Day Shift

CVHCare is seeking a qualified Occupational Therapist to join our team. Home health experience is preferred but not required. This position offers two options: guaranteed hours or non-guaranteed hours.

Responsibilities

• Conduct patient evaluations and develop treatment plans.
• Perform OASIS assessments and related documentation.
• Provide routine therapy, evaluations, discharges, and follow-ups as needed.
• Maintain productivity standards as required for the assigned hours.
• Willingness to travel outside of assigned territory if necessary.
• Compliance with health safety protocols, including treatment of Covid+ patients.

Required Experience / Certifications / Licensure

• Current California Occupational Therapy License
• Basic Life Support (BLS) certification
• Valid Driver's License and auto insurance
• Annual Tuberculosis (TB) screening
• Immunizations: MMR, Varicella, HepB, TDap (10 years), Flu (annual)
• Drug screen and physical examination
