Key Responsibilities

Manage a hospice caseload of up to 20 patients.
Complete all RN Case Manager (RNCM) responsibilities and follow established plans of care.
Provide compassionate, patient-centered hospice care.
Collaborate with the interdisciplinary team, including aides, social workers, physicians, and spiritual caregivers.
Participate in in-person IDG meetings every Tuesday.
Refill medications and coordinate medical supplies.
Address patient and family needs promptly to ensure quality outcomes.
Maintain accurate clinical documentation and care coordination.

Required Qualifications

Active RN license in the State of Nevada.
Hospice, home health, or case management experience preferred.
Strong communication and care coordination skills.
