Physical Therapist - PACE Program - Fort Myers

Job Summary Physical Therapist position at Chapters Health System. Full‑time, Monday‑Friday. Integral member of the inter‑disciplinary team providing short‑term and long‑term care in client homes and facilities. Key Responsibilities Provide and direct physical therapy services. Initiate a physical therapy programme and instruct personnel, family or caregivers in certain phases of treatment. Evaluate and assess patients. Document initial evaluations, progress notes and daily records in participant medical records, including the Plan of Care. Collaborate with other team personnel to achieve well‑rounded care. Knowledge & Skills Excellent communication skills via phone, in person and email. Attention to detail and ability to handle confidential information in compliance with HIPAA guidelines. Required Qualifications Bachelor’s degree required; Master’s program in Physical Therapy preferred, accredited by a recognised organisation. Board Certified Music Therapist (MT‑BC) or eligible to become. Minimum one year experience working with frail or elderly population. Minimum two years experience as a Physical Therapist. Community or home‑health experience preferred. Valid Florida driver’s licence with a clean record. A valid automobile insurance policy. Current CPR certification. Clear level‑2 background check. Educational and Training Requirements Current Florida license as a PT. Certification of completion of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs. Position Specific Requirements Initial and ongoing assessment of patients upon referral from the multi‑disciplinary team. Evaluate patient/family response to therapy and recommend therapeutic functions. Identify problems, develop solutions and set goals related to the patient’s needs. Create treatment plans in consultation with the IDG and the physician, revising the plan of care as required. Review and complete patient assessments and plans of care. Identify equipment to increase patient function and independence. Plan and develop maintenance, exercise, and therapy programmes for participants. Provide education to family members and friends for components of the treatment plan that they can carry out. Document all therapy interventions, assessments, evaluations, treatments and responses to therapy. Oversee duties of Physical Therapy Assistants and PT/OT Aides. Represent the company professionally at all times. Complete all job responsibilities thoroughly, using good judgment and excellent customer service. Seek assistance from the immediate supervisor when unsure of a task. Assist with special projects and other duties as assigned. Compensation Pay range: $76,133.58 – $118,958.71. Background & Drug Testing Employment conditional upon consent to drug and/or alcohol testing and ongoing compliance with the Drug‑Free Workplace Policy.
