Nurse Practitioner

About the Role As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care. In this role, you will work onsite at our Med Center and Boerne clinics located in the San Antonio area and report to our Associate Medical Director. Your Role & Responsibilities Provide comprehensive care to seniors following clinical standards. Collaborate daily with your care team to ensure coordinated, high-quality care. Manage referrals, hospital/SNF coordination, DME, and home health services. Document care accurately and efficiently with support from quality-based coders. Participate in on-call rotation and support growth plans at your center. Show passion for providing value-based care. Required Qualifications Master's Degree in Nursing or completion of a PA program with board certification. Active, unrestricted NP or PA license in the state of practice. Must be screened for TB if selected for this role. Preferred Qualifications 2 or more years of experience in value-based care, primary care or Medicare Advantage settings. Active DEA license and Medicare/Medicaid provider numbers, or ability to obtain them. Schedule & Compensation Scheduled Weekly Hours : 40 hours per week Pay Range : $123,800 - $170,300 per year, plus potential bonus incentive plan based on performance.
