As a Registered Nurse Case Manager , your ability to impact patient care is both recognized and encouraged at every level. Whether through collaborative relationships with your direct supervisor or through structured opportunities in hospital councils and national nursing initiatives, your input will help shape decisions that enhance patient outcomes and the nursing profession.
Job Overview and QualificationsYour primary responsibility as a Case Manager will be to assist patients and their families throughout their healthcare journey. You will coordinate services, connect them with resources, and create care plans tailored to each patient's specific needs. By collaborating with physicians, nurses, and departmental leaders, you will help facilitate safe transitions, ensure effective communication, and maintain high-quality care across the entire continuum.
Your key responsibilities will encompass:
- Assisting patients and families during program orientation by explaining rehabilitation philosophies, Medicare and insurance benefits, discharge criteria, and patient rights.
- Coordinating educational initiatives for patients, families, and caregivers to foster engagement in treatment planning, goal-setting discussions, and family meetings.
- Creating and executing personalized treatment plans that align with the patient’s strengths, needs, and recovery objectives.
- Conducting psychosocial assessments and discharge planning evaluations, thoroughly documenting findings and communicating needs to the care team.
- Working closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary teams to ensure seamless, patient-focused care.
- Upholding the organization’s values of compassion, respect, and excellence in every interaction with patients and families.
- Registered Nurse (RN)
- Bachelor’s Degree, Master’s Degree, or Registered Nurse Diploma
- No travel required
- A minimum of 3 years of relevant experience
