Physical Therapist

Saint Mary of Nazareth Hospital in Chicago, IL is seeking a compassionate, patient-centered Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide evidence-based physical therapy evaluation and treatment to a diverse adult and geriatric patient population across inpatient and, as needed, outpatient settings. You will complete comprehensive assessments, develop individualized plans of care, set functional goals in collaboration with patients and families, and deliver therapeutic interventions aimed at improving mobility, strength, balance, endurance, pain management, and overall quality of life. Working closely with physicians, nurses, case managers, and other rehabilitation professionals, you will participate in interdisciplinary care planning and discharge planning to support safe transitions of care and optimal patient outcomes. Responsibilities include performing standardized tests and measures, educating patients and caregivers on home exercise programs, mobility and safety strategies, documenting all care in the electronic medical record in accordance with regulatory and hospital standards, and maintaining accurate, timely treatment notes and progress reports. You will actively contribute to a culture of safety, quality improvement, and continuous learning by following evidence-based practice guidelines, participating in performance improvement initiatives, and engaging in ongoing professional development.
