RN - Hospice

Pride Health is hiring a Registered Nurse - Hospice for a 13-week contract assignment supporting a leading healthcare organization in Menominee, MI . Responsibilities Include: Coordinate end-of-life care for hospice patients by serving as the primary liaison between patients, families, physicians, the hospice medical director, and the interdisciplinary care team. Conduct comprehensive physical, psychosocial, and spiritual assessments while developing and managing individualized care plans for patients in their homes or residential facilities. Manage pain and symptoms, educate patients and caregivers on disease progression, medication management, and the grieving process, and provide compassionate support throughout the hospice journey. Manage a daily caseload of 5-6 patient visits while ensuring high-quality, patient-centered care. Qualifications: Active Michigan Registered Nurse (RN) license. Current CPR certification. Prior hospice nursing experience preferred. Strong assessment, care coordination, communication, and patient education skills.
