Our Street Health team bridges the gap between vulnerable community members and the care and resources they need. Through field-based outreach, engagement, and Enhanced Care Management (ECM), we help individuals access healthcare, behavioral health services, housing resources, and supportive services that promote improved health outcomes and greater stability. Every day, this team builds meaningful relationships, removes barriers to care, and helps individuals connect to services that support long-term wellbeing.
What Success Looks Like
This is a community-based leadership role that combines staff supervision, program oversight, care coordination, case management, healthcare navigation, and operational accountability. Successful ECM Supervisors are approachable, organized, and committed to coaching staff while building trust with individuals who may be disconnected from traditional healthcare and social service systems. They are comfortable guiding teams in the field, supporting complex member needs, and collaborating closely with healthcare providers, case managers, managed care plans, and community partners.
Success in this role means leading a high-performing ECM team that delivers effective, person-centered care coordination while maintaining strong quality, compliance, and documentation standards. You’ll help strengthen care across multiple systems, support staff growth, remove barriers to service delivery, and ensure members are connected to critical resources that support their health and housing goals. Through strong leadership, collaboration, and follow-through, you’ll help improve access to care, continuity of services, and positive outcomes for individuals with complex medical, behavioral health, and social service needs.
The Role
- Lead daily operations of the Enhanced Care Management program within the Street Health Program.
- Supervise, coach, train, and support ECM Care Coordinators providing field-based care coordination services.
- Coordinate daily team workflows, workload assignments, service priorities, and staffing coverage needs.
- Maintain an ECM caseload and provide outreach, assessment, care planning, resource coordination, crisis intervention, and ongoing member support.
- Provide guidance and support to staff managing complex member situations, safety concerns, barriers to care, and crisis response needs.
- Monitor program performance, productivity, member engagement, documentation quality, and service delivery outcomes.
- Review documentation, conduct chart audits, and support compliance withCalAIM, Medi-Cal managed care, and organizational requirements.
- Track key program metrics and use data toidentifyopportunities for improvement and strengthen program performance.
- Collaborate closely with healthcare providers, behavioral health teams, housing programs, managed care plans, and community partners to support integrated care delivery.
- Promote trauma-informed, person-centered, culturally responsive, and equity-focused care across all aspects of service delivery.
Desirable Candidate Profile
- Bachelor’s degree in Social Work, Psychology, Public Health, Human Services, Healthcare Administration, or a related fieldrequired; or equivalent combinations of education and experience.
- Minimum five (5) years of experience providing outreach, care management, case management, care coordination, or related services to individuals experiencing homelessness, complex medical needs, behavioral health conditions, or other vulnerable populations.
- Minimum two (2) years of supervisory or lead experience, including responsibility for staff coaching, performance management, employee development, and team coordination.
- Experience working with Medi-Cal populations, managed care organizations, interdisciplinary care teams, and community-based service providers strongly preferred.
- Experience utilizing electronic health records, care management platforms, and data reporting systems required.
- Valid California driver's license required.
- CPR certification, or ability to obtain within the requiredtimeframeupon hire.
- Bilingual English/Spanish preferred.
Key Competencies
- Strong knowledge of Enhanced Care Management (ECM),CalAIM, Medi-Cal managed care programs, and integrated healthcare delivery models.
- Knowledge of care management, care coordination, outreach services, and person-centered service planning.
- Strong understanding of trauma-informed care, harm reduction, motivational interviewing, and approaches to serving individuals experiencing homelessness.
- Ability to lead, coach, and develop care coordination teams while promoting accountability, collaboration, and professional growth.
- Ability to build strong relationships with healthcare providers, managed care organizations, community partners, and interdisciplinary teams.
- Strong understanding of quality improvement practices, documentation standards, compliance requirements, and performance management.
- Ability to analyze program data,monitorperformance measures,identifytrends, and implement operational improvements.
- Strong communication, organizational, and follow-through skills.
- Commitment to serving vulnerable populations with compassion, respect, empathy, empowerment, and dignity.
Work Schedule
This is a full-time position that combines administrative leadership, field-based program oversight, staff supervision, and limited direct member care activities. The role is based within the Street Health Program and includes work in community, clinic, office, and outreach settings. While the typical schedule is Monday through Friday, operational needs may require occasional schedule flexibility to support staff, members, community partnerships, and program priorities.
Compensation & Benefits
The anticipated starting pay range for this role is $70,304 to $81,412 annually, paid out bi-weekly. Final placement and starting salary will be determined based on the candidate's experience, qualifications, certification status, and internal equity considerations.
