Work Environment

This full-time position requires an onsite presence, where you will conduct evaluations and treatments in various home settings. As a physical therapist in home care, your office will be where you meet your patients—predominantly their homes—providing a personalized touch that enhances their rehabilitation journey. We maintain a flexible schedule, operating Monday through Friday from 9 AM to 5 PM, allowing for a healthy work-life balance amidst a growing patient base.

Key Responsibilities

• Evaluate patients' functional needs and set individualized goals to facilitate optimal recovery.
• Manage a caseload ensuring you provide one-on-one therapy sessions, fostering strong patient-therapist relationships.
• Utilize evidence-based practices to develop comprehensive treatment plans, integrating various therapeutic techniques and modalities.
• Conduct regular assessments to track patient progress, adjusting care plans as necessary for continued improvement.
• Employ manual therapy skills effectively and develop targeted exercise protocols that enhance mobility and strength.
• Encourage patients to maintain their treatment plans and motivate them throughout their rehabilitation to optimize outcomes.
• Document all patient interactions and treatments accurately, adhering strictly to required regulations and utilizing EMR software, specifically WebPT.
• Collaborate with other healthcare professionals to ensure a holistic approach to patient care and successful rehabilitation.
• Educate patients and their families on self-management strategies, including lifestyle modifications that enhance health outcomes.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• Current and valid state license to practice as a physical therapist.
• Clean professional record with no disciplinary actions against your license.
• Proficiency in manual therapy techniques and exercise protocol development.
• Experience in outpatient orthopedic care with a focus on individualized treatment.
• Familiarity with EMR systems, particularly WebPT, for efficient documentation and treatment tracking.

Preferred Experience

• A minimum of two years of relevant experience in outpatient physical therapy settings, particularly in home care or orthopedic practices.
• Demonstrated success in managing patient caseloads effectively and delivering compassionate, personalized care.
• Experience utilizing innovative therapeutic techniques that enhance patient engagement and outcomes.

Compensation and Benefits
