CLINICAL INTAKE COORDINATOR

General Summary The Registered Professional Nurse (Intake Coordinator) on the Behavioral Health Team uses critical thinking, professional judgment, and leadership to anticipate, plan, direct, and evaluate the care of patients. The role serves as a resource in a specific specialty area, providing screening, case disposition, and referrals for incoming telephone calls, walk‑in inquiries, and inquiries from within Insight Hospital and Medical Center. The Intake Coordinator is administratively responsible to the Director of Behavioral Health and reports to the Department of Psychiatry Chairperson. In all duties, the nurse complies with Illinois Mental Health & Developmental Disabilities Code, hospital policies, procedures, and by‑laws, upholding the institution’s philosophy, mission, and values. All responsibilities are performed confidentially and with a high quality of care toward patients, staff, physicians, volunteers, and guests. Duties and Responsibilities 60% of Time Spent – Screening, Assessment and Referral Screen incoming telephone calls to identify service requests, urgency, patient resources, and appropriateness of Insight Behavioral Health as a provider. Make referrals to other levels of care within Behavioral Health Services when appropriate. Provide resources, contact information, and active linkage for services not offered by Insight Behavioral Health. Coordinate with partnering facilities to review clinical charts with complex medical information and assess patient appropriateness for transfer into the hospital. 30% of Time Spent – Nursing Care Use the nursing process, critical thinking, and professional judgment to provide care for an assigned population of patients, assessing needs to determine care requirements. Plan care and delegate tasks to ancillary personnel based on patient assessment. Demonstrate independent critical thinking and problem‑solving skills. Synthesize information and resources to care for complex, high‑acuity patients. Complete at least 16 hours of continuing education, 30% in addition to mandatory in‑service requirements, within the last year that is specific to daily clinical practice. Participate in interdisciplinary rounds and facilitate discharge planning and patient education. Ensure high levels of quality of care through compliance with regulatory standards, referencing current research related to patient outcomes, and participating in data collection and analysis of ongoing quality monitoring. Assist with admissions and discharges to maintain quality operations of inpatient behavioral health. 5% of Time Spent – Quality Compliance Ensure adherence to regulatory standards and current research related to patient care outcomes, participating in the data collection and analysis process of ongoing quality monitoring. Additional Duties and Responsibilities – 5% of Time Ensure care is provided for all patients utilizing the attributes of Careful Nursing. Practice trustworthy collaboration. Provide health education. Foster safety and comfort with participatory—authoritative management. Carry out other duties related to patient care and unit operations as assigned. Job Specifications Required Knowledge, Skills and Abilities Knowledge Current RN License in the State of Illinois (required) Two years of behavioral health experience preferred Crisis Intervention training required within 90 days of hire; BLS certified Minimum one year of clinical experience on an inpatient behavioral health unit and/or related service line Knowledge of major mental disorders across the age continuum and levels of care Understanding of developmental norms across the age continuum and ability to apply age‑appropriate expectations when engaging patients/families Knowledge of substance abuse and dependency disorders across the age continuum and levels of care Skills Functional status screening skills to select appropriate intensity of care or level of intervention Competence in using the electronic medical record to document and retrieve pertinent patient information and generate required reports Excellent interpersonal skills to engage patients and families in the screening process and develop collegial relationships with professionals inside and outside the organization Communication skills to clearly and concisely transmit essential referral information to clinicians regarding intake requests Abilities Ability to prioritize work assignments, organize and track intake flow, meet turnaround time for call return, and submit required reports Benefits Health, vision, and dental benefits eligible at 30 days following the first of the following month Short and long‑term disability and basic life insurance after 30 days of employment Insight Employees are required to be vaccinated for COVID‑19 as a condition of employment, subject to accommodation for medical or sincerely held religious beliefs.
