About the Role:

The Home Health Occupational Therapist plays a critical role in delivering personalized therapeutic care to patients within their own homes, enabling them to regain independence and improve their quality of life. This position involves assessing patients' physical, emotional, and environmental needs to develop tailored treatment plans that address daily living activities and functional abilities. The therapist collaborates closely with patients, families, and interdisciplinary healthcare teams to ensure comprehensive care and support. By providing education and adaptive strategies, the therapist empowers patients to overcome challenges related to injury, illness, or disability. Ultimately, this role contributes significantly to patient recovery, safety, and long-term well-being in a home health setting.

Minimum Qualifications:

• Valid and current Occupational Therapist license in the state of practice within the United States.
• Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.
• Certification from the National Board for Certification in Occupational Therapy (NBCOT).
• Proven experience or clinical training in occupational therapy, preferably with exposure to home health or outpatient settings.
• Strong knowledge of therapeutic techniques, adaptive equipment, and patient safety standards.

Preferred Qualifications:

• Experience working specifically in home health care or community-based rehabilitation.
• Additional certifications such as Certified Hand Therapist (CHT) or specialty training in geriatrics or neurological rehabilitation.
• Familiarity with electronic health record (EHR) systems and telehealth platforms.
• Strong interpersonal skills with demonstrated ability to educate and motivate patients and families.
• Bilingual abilities or experience working with diverse patient populations.

Responsibilities:

• Conduct thorough evaluations of patients' functional abilities, home environments, and support systems to identify therapy needs.
• Develop and implement individualized occupational therapy treatment plans focused on improving patients' daily living skills and independence.
• Provide hands-on therapy, adaptive equipment training, and education to patients and their caregivers to facilitate effective home care.
• Monitor patient progress regularly, adjusting treatment plans as necessary to achieve optimal outcomes.
• Collaborate with physicians, nurses, social workers, and other healthcare professionals to coordinate comprehensive patient care.
• Document all patient interactions, treatment plans, and progress notes accurately and in compliance with regulatory standards.
• Ensure adherence to safety protocols and ethical guidelines while delivering care in diverse home environments.
