WellBe Senior Medical is seeking a Nurse Practitioner (APP) to provide field-based, in-home primary care in Charleston, WV. You will conduct comprehensive visits and chronic disease management in patients’ homes, collaborating with a multidisciplinary team to deliver safe, high-quality care.
