If this sounds like how you envision your practice, Integrated Care and Consultation is seeking a part-time Family Nurse Practitioner to join our growing team.

This role will work closely with an interdisciplinary team to provide our clients with a compassionate and integrative approach to care.

The Family Nurse Practitioner will:

• Provide health promotion and disease prevention services

• Perform health assessments and physical exams

• Order, perform, and evaluate diagnostic tests

• Diagnose acute, chronic, and complex health problems

• Prescribe medications and non-pharmacological treatments

• Develop and manage treatment plans

• Follow up and evaluate patient status

• Maintain patient records

• Educating patients on preventative care and disease management.

• Collaborate with other healthcare providers to ensure safe and effective medication management.

• Patient, family, and community education and counseling

• Coordinate patient care and make referrals as needed

• Formulating treatment plans for acute and chronic diseases.

• Ordering and analyzing diagnostic tests.

• Managing overall patient care.

REQUIREMENTS:

Must have at least 3 years of experience in a primary care setting.

Master of science in nursing (MSN).

SD CNP license.

DEA license.

Must have graduated from a nonprofit university.

Excellent interpersonal and communication skills.

Professional organization memberships.

Team player.
