Clinical Services Navigator- Registered Nurse - Cancer Hematology Clinic

Nurse NavigatorThe nurse navigator will be in charge of referral management and clinical triage. They would provide centralized oversight of incoming referrals, evaluate referral urgency and scheduling needs.ONCOLOGY EXPERIENCE REQUIREDWhat we offer:Benefits begin on day one of employment.Competitive benefits packageCareer growth and advancement opportunitiesClinical and non-clinical career pathwaysTuition reimbursementApproved employer for the Public Service Loan Forgiveness (PSLF) ProgramWhat You'll Do:Perform comprehensive clinical review of incoming malignant hematology referrals.Prioritize referrals according to established disease-specific triage criteria.Ensure patients are matched with the appropriate malignant hematology subspecialist.Facilitate expedited scheduling of urgent and high-acuity cases.Coordinate collection of required medical records and diagnostic information prior to consultation.Serve as a clinical liaison between referring providers, scheduling teams, and treating clinicians.Improve referral throughput and reduce administrative burden on clinic nurses, physicians and advanced practice providers.Support timely patient access and enhance the overall patient and referring provider experience.What We're Looking For:Education: 4 Year / Bachelors Degree Required.Experience: Three to five years of nursing experience, required.
