Adelante Healthcare is seeking a Behavioral Health Case Manager to oversee a caseload of clients, assess needs, and develop and review service plans with input from internal and external resources. The role emphasizes quality patient care, cultural responsiveness, and collaboration with a multidisciplinary team.
