Human Health Project (HHP) is a holistic health literacy charity. Our mission is to improve the health of the underserved and vulnerable through holistic information, education and advocacy. Our objective is to empower people to manage their own health with improved outcomes and no medical errors. About Human Health Project (HHP)
Human Health Project is a global holistic health literacy charity building a Unified Patient Experience (UPE)—a platform and service model that helps patients navigate healthcare, improve health literacy, and access care, education, information and advocacy.
We are now launching a reimbursable care delivery model under Medi-Cal (CalAIM), Medicaid and Medicare , leveraging a trained volunteer workforce.
HHP is building a national model for Community Health Worker (CHW)-driven care delivery , focused on:
- Health literacy
- Care navigation
- Social determinants of health
The Role
We are seeking a Care Delivery & Workforce Lead to operationalize HHP’s volunteer workforce into a structured, high-quality care delivery system .
Key Responsibilities
- Design and manage CHW workflows:
- Patient navigation
- Medicare / Medi-Cal support
- SDOH referrals
- Build and manage:
- Volunteer tiers (Tier 1 / Tier 2)
- Care delivery teams
- Develop training programs:
- CHW readiness
- Documentation standards
- Assign volunteers to pilot patient cohorts
Ideal Background

- Healthcare operations or care coordination
- Volunteer or workforce management
