Job Title: Registered Nurse Profession: RN Specialty: Extended Care Duration: 13 weeks Shift: Day Hours per Shift: 12 Experience: Minimum years of experience required License: Current license as a Registered Nurse in the state of practice is required Certifications: BLS - American Heart Association Must-Have: - Ability to read and write at a level to fulfill documentation requirements - Physical ability to perform required tasks including lifting objects over 100 lbs and frequently lifting/carrying up to 50 lbs Description: The RN utilizes an interpersonal process that uniquely focuses on the total individual and their role in their family, culture, and society. The RN utilizes the nursing process, which includes assessment, diagnosis, planning, implementation, and evaluation. The RN fulfills a resident advocacy role by ensuring the resident and/or their representative has adequate information to make health decisions. They coordinate total care and work with the resident to achieve or maintain their highest practicable level of function. The RN is guided by the substantial specialized knowledge of nursing based on physiological and psychological sciences. They ensure that the quality of resident care meets legal and professional standards and act as a role model to other members of the healthcare team. This position requires the full understanding and active participation in fulfilling the mission, vision, and values of the organization. It is expected that the associate will demonstrate behavior consistent with core values and support goals and direction of the quality improvement process. The RN completes holistic, age-specific admission history, physical assessment, and nursing plan of care on all assigned residents. They analyze and incorporate clinical data to formulate an individualized plan of care. This includes identifying and documenting residents at risk for nutritional and functional status. They identify and report signs/symptoms of neglect or abuse related to individuals. The RN sets priorities for and reviews the daily plan of care in collaboration with residents and their families. Delegation to team members is based on resident needs, continuity of care, and staff knowledge. They communicate with physicians to ensure coordination of medical and nursing plans of care. The RN demonstrates competence in performing technical skills and manages urgent/emergent resident care situations. They show knowledge and technical competence in the preparation and administration of medications and IVs. Documentation of nursing interventions, residents' responses to care, and resident outcomes is required. They evaluate data to revise the plan of care to meet changing needs of the resident. They document resident/family learning needs and provide education based on assessment. The RN identifies and documents discharge planning needs and coordinates or delegates discharge activities. Maintaining knowledge of regulations is crucial to ensure documentation supports compliance. The RN incorporates resident needs and preferences into organized and efficient delivery of care by a team. They supervise, coach, counsel, and evaluate the performance of assigned support staff.
