Description

• Conduct patient assessments in both inpatient and outpatient settings to develop personalized treatment plans.
• Implement therapeutic interventions aimed at enhancing mobility, building strength, cultivating balance, and improving overall function.
• Collaborate seamlessly with nurses, physicians, case managers, and other rehabilitation experts to enhance patient outcomes.
• Educate patients and their families on setting therapeutic objectives, planning for discharge, and performing home exercises effectively.
• Maintain thorough documentation of evaluations, treatment processes, and patient progress in line with both facility standards and regulatory policies.
• Participate actively in care team discussions and interdisciplinary meetings to ensure synchronized, comprehensive patient care.

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution.

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred
• Any specialized certifications (e.g., in neurology or geriatrics) are beneficial but not mandatory.
